Current medications/side effects	Tambacor; tiredness, vision and percepyion problems, IQ reduction while on med. sleep problems cradizem; nausea, tiredness
Past medications/side effects	tenormin; tiredness
History of surgery	both shoulders, rotator cuffs- full retractions, reattached with s steek anchors third degree burns, skin transplants pva early July, 2002 second pva-pvi scheduled for September 12, 2002
Future plans	To return to normal if opperation works
